Hydroxyzine and QT prolongation: what the warning actually means

Almost fifteen million hydroxyzine prescriptions were filled in the United States in 2024, by about 4.7 million people [3] — mostly for anxiety, often chosen precisely because it isn't a benzodiazepine and isn't habit-forming. And its label carries a cardiac warning that most of those 4.7 million have never had explained to them.

What the label says, verbatim

From the current FDA prescribing information, under Precautions [1]:

"QT Prolongation/Torsade de Pointes (TdP): Cases of QT prolongation and Torsade de Pointes have been reported during post-marketing use of hydroxyzine. The majority of reports occurred in patients with other risk factors for QT prolongation/TdP (pre-existing heart disease, electrolyte imbalances or concomitant arrhythmogenic drug use)."

And under Contraindications, one sentence that goes further than caution:

"Hydroxyzine is contraindicated in patients with a prolonged QT interval."

Three things worth unpacking, because each is routinely garbled elsewhere.

"Post-marketing reports" is a specific kind of evidence. These are case reports filed after approval — not controlled-trial findings with a denominator. They can establish that something happens; they cannot establish how often. The label is honest about this, and so should any page quoting it.

"The majority occurred in patients with other risk factors" cuts both ways. It means a healthy adult with no cardiac history, normal electrolytes and no interacting drugs is not the population the reports describe. It also means the risk factors are common enough — heart disease, low potassium, a long list of QT-prolonging co-medications — that the qualifier applies to a lot of real patients. Neither reading erases the other.

A contraindication is not a caution. For people with an already-prolonged QT interval, the label doesn't say "use carefully" — it says don't. If you've ever been told you have long QT, that sentence is the whole article.

What QT prolongation actually is

The QT interval is the stretch of the heartbeat's electrical cycle during which the heart's main pumping chambers reset. Some drugs lengthen it. A longer interval usually causes nothing at all — and occasionally sets up torsade de pointes, a chaotic rhythm that can cause fainting, seizures, or cardiac arrest. That's the entire logic of every QT warning in medicine: the interval itself is silent; the rare downstream arrhythmia is the harm.

Which is why the practical warning signs are concrete rather than vague: fainting, seizures, or sudden severe palpitations — especially early in treatment, after a dose increase, or when a new medication joins the list — are the events that warrant immediate care.

The 2015 fork: Europe capped the dose, America didn't

In February 2015, the European Medicines Agency's safety committee finished a formal review of the cardiac signal and concluded hydroxyzine carried "a small but definite risk of QT interval prolongation and torsade de pointes" [2]. Europe acted structurally: a maximum adult dose of 100 mg per day (50 mg in the elderly where use can't be avoided; weight-based caps in children), lowest effective dose, shortest possible duration, and avoidance in patients with rhythm risk factors or interacting drugs.

The FDA's response to the same signal was the label language above — a warning paragraph and a contraindication, but no dose cap. US prescribing information permits doses well above the European maximum.

We are not the arbiter of which regulator got it right; dose caps have costs too, and a later analysis found the EU changes measurably shifted prescribing [4]. What we'd argue is simpler: a person deciding about this drug deserves to know the fork exists. If your prescription is for 200 mg a day, it is entirely fair to ask your prescriber, "Europe caps this at 100 — what makes the higher dose right for me?" That's not a challenge; it's the informed-consent conversation working.
